How is FSGS and extreme fatigue linked with each other?

In most of the cases, FSGS patients suffer from extreme fatigue due to renal anemia which is a medical condition in which red blood cell account in blood is less than the reference value. Red blood cell is an important component of our blood and also the major reason to make blood red. Red blood cell circulate with blood and other substances in the blood and meanwhile carrying oxygen to our body cells. In this way, we feel energic.

In cases of FSGS, kidney tissues are injured, which can lead to the decrease of EPO (erythropoietin), a must for the synthesis of red blood cell, as EPO is normally secreted by kidney. Due to EPO insufficiency, red blood cell account decreases. Under such a condition, oxygen can not be brought to body tissues timely and patients become tired easily.

Renal anemia is the major cause of extreme fatigue in FSGS, but in some cases, patients may become fatigued for some reasons. What causes protein spillage in FSGS?

In our kidney, glomerular epithelial cells and renal tubular epithelial cell play a very important role to keep protein in blood. Glomerular epithelial cells preserve protein via mechanism barrier which only allow some small molecular protein to pass through. These proteins are finally reabsorbed by renal tubule. In FSGS, when glomerular epithelial cells are injured, large amounts of protein, including protein with small molecular weight and large molecular weight, will get into urine. In such a case, patients are always told to have massive proteinuria. However, if glomeruli can work well, and renal tubular epithelial cells are damaged seriously, small molecular protein filtered by glomeruli will finally get into urine. Therefore, through special urine test, not routine urine test, we can know clearly about whether patients’ glomerular epithelial cells are damaged or their renal tubular epithelial cells are injured. In general, the more serious kidney damages are, the more serious protein spillage is.
