Cyclosporine for Recurrent FSGS

Question: My son has FSGS. Since he was 2, now 7. It has been controlled with cyclosporine. Recently doctors tried him with cellcept, which didn't work, he had a relapse a few weeks ago. He is now back on cyclosporine and being weened of prednisolone. He went back to being controlled, but today he had 3+ protein in urine. I am worried cyclosporine is not going to work anymore.

Conventional treatment for Nephrotic Syndrome is to use steroids such as prednisone to reduce protein leakage. But for people with Nephrotic Syndrome caused by FSGS, they usually have bad response to steroids. In such case, other immunosuppressants, such as cyclosporine are used together with steroids to suppress the immune system and inflammatory reaction in the kidneys. Though they help the patients eliminate symptoms, they have many side effects if used for a long period of time. Besides, cyclosporine has renal toxicity, so you should pay attention when using such kind of medicine to avoid further kidney damage.

But from what you described, we can see that there is frequent relapse with cyclosporine. I think your son is steroid-dependence. That is to say, once he reduces the dosage or stop the medicine, the disease comes back again. If you son use cyclosporine for years, it may affect his physical and psychological health.
